When Eczema Gets Infected: Staph and Herpeticum

Eczema is hard enough without complications. The itching, the sleepless nights, the constant flare and repair – it all takes its toll. But there’s a hidden risk that many people aren’t told about. Infection.

An eczema flare can tip from “difficult” to “dangerous” surprisingly quickly. Two culprits are most often to blame: bacteria, particularly Staphylococcus aureus (“staph”), and viruses such as herpes simplex, which causes eczema herpeticum. The challenge is that they can look alike, sometimes show up together, and often confuse even experienced doctors. Here’s what happens when eczema gets infected.

Why infection is part of the eczema picture

You may have been told that infection is rare in eczema. Most people with eczema carry staph bacteria on their skin. Most of the time, it causes no trouble. But when skin is scratched, broken, or the barrier is weakened, those bacteria can flare into infection.

Herpes simplex virus is another story. The same virus that causes cold sores can spread quickly through eczema patches. This condition, known as eczema herpeticum, can become a medical emergency if not treated fast.

So, infection isn’t an “if.” It’s part of the eczema picture – and recognising it matters.

How to spot the difference

Staph infection often shows up as:

  • Yellow crusts or oozing
  • Redness that spreads
  • Swelling, heat, or worsening pain
  • Sometimes fever or feeling generally unwell

Eczema herpeticum usually appears as:

  • Clusters of small blisters that look like tiny holes punched into the skin
  • Rapid spread, often very painful
  • Fever, chills, or feeling unwell
  • Crusting as the blisters break down

One important fact: the two often appear together. This is why standard creams or antibiotics sometimes don’t work – because they only tackle one half of the problem. Doctors can run tests to confirm whether herpes or bacteria, or both, are involved, and then treat appropriately (1–4).

The hidden clue in food

There’s also an intriguing link between nutrition and herpes. The virus relies on an amino acid called arginine to grow. Lysine, another amino acid, competes with arginine and can slow viral replication.

This explains why some people notice that foods like chocolate, nuts, or seeds – all high in arginine – seem to trigger cold sores or flare-ups, while foods richer in lysine, such as poultry and fish, can have the opposite effect.

This isn’t the main driver of infection, but it shows how many layers of biology play a role in why some people develop viral complications and others don’t.

The bigger picture

The most important thing to remember is this: infection is not a sign you’ve done something wrong. Eczema skin is biologically different. It is more open to microbes, which makes infections more likely.

What you can do is notice changes quickly, act early, and understand that infection is about biology, not blame.

We have been working with Jessica for a while now for my son’s eczema and many food intolerances. We have come a long way with him tolerating a lot more foods whilst previously being on such a limited diet. We have seen a huge difference in his skin too. One thing we love about working with her is that she is highly knowledgeable about how the gut works, what food works well and how to carefully introduce food to increase tolerance. Jessica was a fantastic support when my 1 year old developed severe eczema.
